Referral of patients with proven cutaneous melanoma to TBH

All patients with a confirmed histological diagnosis of cutaneous melanoma should please be referred to the TBH Dermatology department by sending an email to the following address: TBHMelanoma@gmail.com. The referral should include the following information:

Name of referring doctor:
Patient name:
Patient DOB:
Patient address:
Telephonic contact details of patient:
Brief history and description of the index lesion(s)
Histology report proving cutaneous melanoma (attachment)

All referrals will be reviewed regularly by the dermatology team under the supervision of Dr Willie Visser. Feedback will be provided via email regarding the need for further appointments, additional information required and planned investigations.
